New Patients
To book an appointment at Avondale House Dental Surgery as either an existing or new patient please give our reception team a call on 01889 582 080 or visit our website to book your appointment online. As registration under the NHS no longer exists we welcome new patients from all geographic areas. We have regular patients from all over the UK. You do not have live local to us to use our services and welcome patients from near and far that are having difficulty in finding a dentist or simply wishing to change to our surgery.
If you are exempt from NHS charges please bring with you any proof that you have that you are exempt or entitled to reduced cost NHS treatment. If you are unsure please check and satisfy yourself before you are seen that you are exempt or entitled to reduced cost treatment as our staff are unable to check or offer advice on this.
When you first visit our practice, we will check your teeth and mouth, assess the health of your gums and your mouth will be screened for oral cancer, as well as a check of your jaw joint and, if necessary, take x-rays. We will also take a detailed medical & dental history from you.
After listening to your concerns and discussing treatment options we will create a personal treatment plan based on the information from this visit.
In order to get the most out of your first visit it would be useful if you could complete the medical history form and the smile questionnaire and bring these with you. These can be printed from the links below.
